Preparing for your first appointment

Complete paperwork emailed to you prior to attending your first appointment
Arrive 10-15 minutes early for your first visit.
Make a list of questions you’d like to ask during your first meeting.
Bring identification with you.
Please call our office ahead of time if you have any questions about the intake forms emailed to you (414) 299-8121.
Prepare to talk about your medical history, previous treatments, current medications, and other relevant details.
Make a list of any symptoms the therapist should be aware of. What has recently occurred to contribute to the symptoms (if known)? How long have you been dealing with this problem? What are you unable to do as a result of it?
Bring along or have access to any recent blood tests or reports from your provider to discuss with your practitioner.

ONE MOST COMMON QUESTIONS WE GET IS DO WE NEED A DOCTOR’S REFERRAL TO SEE YOU ESPECIALLY IF THIS IS FOR PAIN OR AN INJURY. FOR FUNCTIONAL MEDICINE TREATMENT A REFERRAL IS NOT NECESSARY AS THESE TYPE OF SESSIONS ARE ALL OUT OF POCKET AND NOT GOVERNED BY WHAT INSURANCE COMPANIES AGREE TO PAY FOR. BUT FOR PHYSICAL THERAPY, PATIENTS OFTEN THINK THEY NEED A PROVIDER REFERRAL TO BEGIN. THOUGH WE VERY MUCH ENJOY WORKING WITH YOUR PROVIDER, FOR MANY/MOST CASES YOU DO NOT NEED AN ORDER TO SEE US.

What to expect at New You Health and Wellness during your first appointment:

Arriving at our physical therapy and functional medicine clinic is not like arriving at your doctor’s office so here is what you can expect.

You will arrive and make yourself comfortable in our waiting area. If our front office person is available please check in with the front desk. If not, please text 414 299 8121 to let the practitioner know you have arrived.
After that, you’ll have your initial evaluation with one of our physical therapists or functional medicine specialists. We will ask about your medical history, current and past symptoms, illnesses, or injuries/situations that led up to the reason you are seeing us. Please be open and honest with us so that we can better understand your problem and provide you with the best possible solution!
Our Functional Medicine practitioners will fully evaluate your history, current supplementation program, medications, and current complaints. Our physical therapists will also assess your movement, strength, and flexibility and pain during your appointment.
Based on the findings of your evaluation, our natural practitioner will create a plan for you. The goal will be to recover as quickly as possible, while achieving meaningful results each step along the way. On the first day, depending on the type of consultation you scheduled you may receive some treatments to aid in your recovery; however, your following visit will include a complete treatment session.
For our Functional Medicine clients you will start with a personalized program upon which appropriate interventions and assessments will be layered according to your time frame. For example some patients like to order all tests right away, others would rather spread them out. Same with the rest of our program…, our practitioners will begin to streamline your supplementation, eating plan, exercise and activity recommendations at the pace that is comfortable to you. Your preferences are important to us as everyone is different.
Our Wauwatosa, WI Functional Medicine specialists and Physical therapists will record their findings and treatment plan, and they can but in communication with your referring physician or another medical expert of your choosing if you choose.

You will arrange the remaining sessions at the front desk or with your practitioner before leaving your initial appointment at New You Health and Wellness. Our practitioner will determine the number of treatment sessions needed and will discuss this with you at your first appointment.
